In the final kilometre the 2005 world champion blew past fellow Belgian Greg Van Avermaet, who had done brilliantly to work his way back into the peloton after suffering a crash near the end of the 147.5 kilometre trip from Camel Race Track to the Qatar Foundation.
Boonen then easily held off Italian sprinter Napolitano by more than a full bicycle's length to claim a second successive personal victory and a third straight win for the Quick Step team.
Van Avermaet was relegated to fourth by Boonen's lead-out man and team-mate Steven De Jongh, while Christopher Sutton continued to impress with a fifth-place showing.
The 27-year-old Tour de France green jersey Boonen easily retains the overall lead heading into Wednesday's 131.5-km fourth stage from Khalifa Stadium to Al Khor Corniche.

Tom Boonen's Quick Step won the opening team trial of the Tour of Qatar for the second successive year.
Matteo Tosatto took the opening leader's jersey after crossing the line first for the victors, who caught a piece of luck when Kevin Hulsmans' rear tubular exploded - but only after he had completed the race.
Team Slipstream, led by British time trial specialist David Millar, claimed second in the six kilometre time trial through Doha.
