Aguero, who scored the dramatic late winning goal when City beat QPR on the final day to secure the crown two years ago, had been doubtful with a groin injury.
He will now take his place in a fully-fit squad to face the Hammers at the Etihad Stadium, with City knowing a draw should be enough to clinch a second title in three seasons.
Manager Manuel Pellegrini, speaking at his pre-match press conference, said: "The whole squad is fit for Sunday. It is very important, always for a manager, if you have your whole squad and choose from all your players."
