Italy took a surprise 14-12 lead into the break as Aidan Guerra crossed after just seven minutes. Joshua Manatellato neatly dispatched the conversion but the hosts soon levelled as Ben Evans was on hand to ground the ball following some poor Italian defending. Lloyd White's conversion levelled the scores inside the Millennium Stadium.
Guerra's converted try put the visitors in front after 20 minutes before Mantellato kicked to nudge Italy 14-6 ahead.
Rhys Williams grabbed a try late in the first half to reduce the deficit.
Elliot Kear scored with just seconds on the clock following the break as the hosts took a 16-14 lead, though the conversion attempt flew wide.
Mantellato and Chris Centrone then crossed for Italy, seemingly putting the game to bed with the score standing at 26-16. Wales pushed in an attempt to get back into the match but were caught out in the final minute as Mark Minichiello crossed out wide.
