Chelsea can take a huge stride towards the Premier League title if they beat West Ham in Monday's London Stadium derby.

Antonio Conte is closing in on a title triumph in his debut season in England, and needs 29 points from the Blues' remaining 12 games to be sure of lifting the trophy.

Chelsea head to east London to take on Slaven Bilic's Hammers having lost a tempestuous EFL Cup clash there earlier this season as both sets of supporters clashed in the stands at the end of the game.

All eyes will be on the match to ensure a repeat of the violence is avoided, while Conte will also be hoping for a better result on the pitch this time around as he hunts the title.

TEAM NEWS

Andy Carroll is battling to be fit for Monday night's clash after missing the Hammers' 1-1 draw at Watford with a groin injury, though Slaven Bilic remains confident the striker will be available.

Diafra Sakho remains sidelined with a back injury, while Angelo Ogbonna and Gokhan Tore are both out for the rest of the season following knee injuries.

Chelsea have no fresh injury concerns as Antonio Conte boasts a fully-fit squad to choose from.

The only real decision the Italian coach will have to make is whether to play the in-form Cesc Fabregas alongside N'Golo Kante in midfield.

Conte may elect to take a more cautious approach by selecting the more defensively-minded Nemanja Matic having lost at this stadium once already this season.
